Southern Cross Care Community Services began its operations in 2002. Southern Cross Care Community Services provides services throughout South East Queensland with service outlets in Brisbane, on the Gold Coast, Ipswich and Logan.
Our belief is that everyone has the right to choose to live in his or her home. This is part of our commitment to value and respect human life and to support our clients to get the most out of their lives now. Our community services provide support for those more elderly who choose to stay in their own homes longer, for those with disabilities and other clients needing in-home support.

Coordination and care staff have extensive experience in Client Directed Care (CDC) drawn from the relationships we have with clients under individualised funding allocations within disability funding arrangements, e.g. Lifestyle Packages. In these relationships we understand the relevance and importance of working in collaboration with people with disability and their families to optimise life choices and realise the outcomes they identify for themselves. Southern Cross Care Queensland works within a strength based care framework and our philosophy, values, policies and supporting tools directly facilitate active engagement of consumers and their families.
Southern Cross Care Queensland has a comprehensive portfolio of marketing material to promote and inform current and potential care recipients and their representatives about the organisation and the services and programs being delivered. Our information is readily available throughout a wide network including hospitals, GP surgeries, ACAT, community network meetings as well as local advertisements and our website.

Assessment, Goal Setting and Care Planning: Assessment is an important part in the process of individualising the Service Plan for our clients and/or their carers. It is essential that the assessment be flexible, supportive and actively engages with our client, carer and/or their representative and other members of the multidisciplinary team involved in their care. The assessment provides information that is critical to the development of the client's goals (long term and short term) and how we might assist them in achieving those goals. It is also an important part of identifying the client’s strengths and needs, while at all times respecting their lifestyle choices.
By working with the client to assess their physical, psychosocial and functional status, a Service Plan can be developed that acknowledges their choices, goals, strengths and meets their needs. The assessment process at Southern Cross Care (Qld) is flexible and appropriate to our client’s needs.
We operate under Government funded schemes for in-home support and disability support services, plus we can provide a fast and effective ‘fee for service’ structure. This covers preventive services, such as:
