Southern Cross Care residents, clients, family members, volunteers, and staff can all nominate a Southern Cross Care team member for this award.

Any member of any Southern Cross Care community (staff, volunteers, residents, clients, family members and volunteers) can submit a nomination.
As many as you like. Please submit one nomination form per nominee.
All nominations are to be submitted via the online form. Please complete all questions.
Provide as much detail as possible about why you are nominating the person and include examples of how the person goes above and beyond in their role, and how they make a difference to you or the people around them. Please use the correct full name of the person you are nominating, so we can make sure the right person is recognised for their good work.
